Poor decisions set the stage for Deepwater Horizon disaster, panel finds

BP’s cost concerns appear to have resulted in shoddy risk management, which contributed significantly to the explosion that killed 11 and unleashed the Gulf of Mexico oil spill, a federally convened committee says. Failure to manage the risks of a complex well and to learn from an earlier narrowly missed disaster contributed significantly to the Deepwater Horizon disaster in the Gulf of Mexico, a panel investigating the BP oil spill said Wednesday.
